Beschreibung	Both an additional text field and an additional combo box can be specified as mandatory fields on the entry form. We would also find this option for the additional checkbox very useful, as we would like to request confirmation that you have read and understood the disclaimer in the entry form. It should only be possible to send the entry form if the necessary checkbox has also been checked
